UpDog Gym - Pack Perks Membership Agreement

This contract is between UpDog Gym (the "Business") and the “Client” signed below.

The Client is joining UpDog Gym’s Pack Perks Membership

Terms

This Membership Agreement ("Agreement") is entered into by and between UpDog Enterprises LLC ("UpDog Gym") and the individual member who has enrolled in the Pack Perks membership program ("Pack Member"). By signing this Agreement, the Pack Member acknowledges and agrees to the terms and conditions outlined below.

1. Membership Overview
The Pack Perks membership program is designed to provide Pack Members with exclusive discounts on gym services from UpDog Gym, as well as additional benefits from our network of affiliate businesses. In exchange for the benefits provided by the Pack Perks program, the Pack Member agrees to comply with the terms and conditions set forth in this Agreement.

Benefits of the Pack Perks membership include:
- Discounted Sessions: Pack Members will receive discounted rates when purchasing 4 or more sessions from UpDog Gym per month.
- Affiliate Business Discounts: Pack Members will receive exclusive discounts with our network of affiliate businesses. These may include discounts at pet stores, grooming services, pet supplies, and other various businesses.
- Bonus Perks: Pack Members will receive occasional bonus items from UpDog Gym. These may include gym merchandise, pet-related products, or special offers exclusive to Pack Perks members.

2. Membership Requirements
Minimum Monthly Sessions: To maintain eligibility for the Pack Perks discounted rate, Pack Members must purchase a minimum of four sessions per month. Additional sessions purchased for the same month will also receive the Pack Perks discount.

Invoicing and Payment:
- Membership is billed as a monthly subscription that renews on the same day each month. Payment is due by the 1st of every month for continued participation in the Pack Perks program.
- Any changes to session dates or times must be made before the 1st of the following month. Once the subscription is renewed, session appointments are subject to UpDog Gym’s cancellation policy.
- If payment is not received by the 1st of the month, all sessions for that month will be charged at the regular, non-discounted rate.

3. Rescheduling, No Shows and Cancellations
- Rescheduling: Pack Members are allowed to reschedule one session per month. Rescheduling requests must be made at least 48 hours in advance of the scheduled session. Requests to reschedule with less than 48 hours notice will be treated as cancelations.
- No Shows/Cancellations: No shows and/or sessions canceled by the Pack Member are non-refundable. In the event that a session needs to be cancelled by UpDog Gym, the Pack Member may choose between rescheduling the session or receiving a refund for the cancelled session.

4. Loss of Pack Perks Membership
- Excessive Cancellations or Reschedules: If a Pack Member needs to reschedule or cancel more than one time per month, they may lose their Pack Perks Membership eligibility.
- No Shows may result in termination of membership.

5. Membership Term and Termination
- Term: This Agreement will remain in effect on a month-to-month basis, and the membership will automatically renew upon receipt of subscription payment.
- Termination by UpDog Gym: UpDog Gym reserves the right to terminate this Agreement at any time for any reason.
- Termination by Pack Member: The Pack Member may terminate their Pack Perks membership at any time, but will not be entitled to a refund for any unused sessions.

6. Additional Terms
Amendments: UpDog Gym reserves the right to amend this Agreement at any time. Any amendments will be communicated to the Pack Member in writing, and continued use of the membership will constitute acceptance of the revised terms.

7. Acknowledgment
By signing below, the Pack Member acknowledges that they have read and understood the terms of this Agreement and agree to abide by all the conditions set forth herein.
